This machine was made by GolfMechanix for Golfsmith. The same machine is still being sold by both GolfMechanix, and Golf Works. Current price at Golf Works is $619.99 + tax + shipping.
It works well, and includes the original instruction guide.
As mentioned in the Item Description, the power pack has one prong that's been broken (through a hole drilled in the prong) which results in the prone being slightly shorter than the other. This has never proven to be a problem with my usage, however. I just don't want anyone to be surprised.
The top clamp has a torque limiting feature to assure consistant results. You can do full shaft profiling, or just simple butt flex checks easily.
